Male fertility testing: Why a semen analysis alone may not be enough

What a normal semen analysis may conceal

“The semen analysis is normal.”

For many couples, this is a sentence that should bring relief. Yet sometimes it raises even more questions. The months pass, pregnancy does not occur, and the same question returns: Why?

In approximately half of cases, difficulties with conception are associated with a male factor. Male fertility testing is therefore an important part of the couple's diagnostic assessment.

Until a few years ago, answers were sought primarily in what could be seen under a microscope. How many sperm a man had. How they moved. What they looked like.

However, the way we view male fertility has changed significantly.

We now know that fertility cannot be described by a single number or one result. It is the outcome of many interacting factors – from sperm production and quality, through their functional abilities, to the fertilisation process itself. If any of these steps does not work properly, the path to natural conception may be more difficult.

This is precisely why even a normal semen analysis may be only the beginning of the search for answers.

A sperm may look normal under a microscope while still carrying damaged genetic information. It may be sufficiently motile, but its ability to fertilise an egg may be affected by other factors. Or it may reach the egg but be unable to complete the fertilisation process successfully.

These are precisely the properties that a routine semen analysis does not assess.

Why a semen analysis alone does not always provide a complete answer

A basic semen analysis remains an important first step in male fertility testing. It primarily assesses sperm count, motility and morphology.

Modern diagnostics, however, no longer rely on a single result. They bring individual pieces of information together and also assess the quality of the genetic information carried by sperm, possible effects of the immune system and the sperm's actual ability to participate in fertilisation.

This more comprehensive perspective may explain cases that previously remained without a clear answer.

Which tests can reveal hidden causes of male infertility

If a basic semen analysis does not provide an explanation, further tests may help identify causes that routine sperm examination cannot detect.

One of these is the sperm DNA fragmentation test, which evaluates the quality of the genetic information stored in sperm. Even when sperm count and motility are normal, DNA damage may be one of the factors affecting the ability to conceive or embryo development.

Another option is the MAR test, which indicates the presence of antibodies against sperm. These may negatively affect sperm movement or their ability to reach the egg.

Testing the function of the CatSper channel may also be highly beneficial. This channel plays an important role in hyperactivation of the sperm flagellum and the sperm's ability to penetrate the egg actively.

The right question leads to the right solution

A comprehensive male fertility test does not merely provide more results; above all, it helps explain more clearly why conception has not occurred.

This makes it possible to choose the next step according to the couple's specific situation. For some couples, it may be appropriate to continue trying to conceive naturally or to use intrauterine insemination (IUI). For others, it may be more effective to proceed directly to IVF treatment.

The decision is therefore based not only on a single result, but on the most accurate possible understanding of the cause.
